Louisville Physical Therapy was started in January 2008 by Hank Lewis, PT. Hank has been providing therapy in the Louisville area since 1993. He has compiled a group of committed professionals that will give you a feeling of comfort and confidence.
Louisville Physical Therapy has an open door policy. Anytime a past patient has a question or concern, they can feel free to stop in and talk to a member of the staff. We look forward to working with you and getting you on the road to recovery.

Hank Lewis, P.T.
Hank is a 1988 graduate of Louisville High School and a 1992 graduate of The Ohio State University. Hank lives in Louisville with his wife Dina and three children Paige, Kyle and Shelby. Hank has been practicing Physical Therapy in the Louisville area since 1992. In his spare time, he enjoys traveling around the country participating in Field Trials with his German Shorthair dogs.
Krista Kappas, D.P.T.
Krista graduated from Columbiana High School in 2007. She received a Bachelor's degree in Exercise Science from Mount Union and then a Doctorate of P.T. in 2014 from Youngstown State University. Krista resides in Louisville with her husband Chris, sons' Coby and Shepard, and daughter Lily. In her free time, she enjoys spending time with family and friends and dog, Ace.
Daren Harsh, P.T.A.
Daren graduated from Louisville High School in 1991. He graduated from Stark State College in 1998 with an Associates of Science degree as a Licensed Physical Therapist Assistant. Daren enjoys spending time with his wife Joy and two children Joslyn and Drayden. He also enjoys working outdoors and traveling with his family.
LAURA PATTERSON, P.T.A.
Laura graduated from Tallmadge High School in 2009. She graduated from Stark State College in 2017 with an Associates of Science degree as a licensed Physical Therapist Assistant. Laura resides in Springfield Township with her husband, Robbie, and her two daughters, Sayla and Charlotte. In her free time, Laura enjoys camping with her family, riding bikes and fishing.
PAIGE AVON, P.T.A.
Paige graduated from Claymont High School in 2017. She graduated from Stark State College in 2020 with an Associates of Science degree as a Licensed Physical Therapist Assistant. Paige resides in Dennison with her boyfriend, Andy, dog, Malcolm and cat, Millie. In her free time, Paige enjoys traveling and spending time with friends and family.
Jill Smith, Secretary/Office Administrator
Jill graduated from Louisville High School in 1994. She attended Stark State College for executive secretary. Jill and her husband Cory reside in Louisville. They have 4 children - Cora, Sydney, Tapanga and Wyatt, and new grandson Jameson. In her free time, Jill enjoys working in their garden, going to Wyatt's sporting events (football and wrestling) and spending time with family and friends. Jill has also been an active 4-H advisor for 16 years, volunteers for the Stark County Dairy Committee and is the Chairperson.
