Louisville Physical Therapy was started in January 2008 by Hank Lewis, PT. Hank has been providing therapy in the Louisville area since 1993. He has compiled a group of committed professionals that will give you a feeling of comfort and confidence.
Louisville Physical Therapy has an open door policy. Anytime a past patient has a question or concern, they can feel free to stop in and talk to a member of the staff. We look forward to working with you and getting you on the road to recovery.

Hank Lewis, P.T.
Hank is a 1988 graduate of Louisville High School and a 1992 graduate of The Ohio State University. Hank lives in Louisville with his wife Dina and three children Paige, Kyle and Shelby. Hank has been practicing Physical Therapy in the Louisville area since 1992. In his spare time, he enjoys traveling around the country participating in Field Trials with his German Shorthair dogs.
Daren Harsh, P.T.A.
Daren graduated from Louisville High School in 1991. He graduated from Stark State College in 1998 with an Associates of Science degree as a Licensed Physical Therapist Assistant. Daren enjoys spending time with his wife Joy and two children Joslyn and Drayden. He also enjoys working outdoors and traveling with his family.
Tracy Wondracek, P.T.A.
Tracy graduated from Louisville High School in 1997. She graduated from Stark
State College in 2001 with an Associates of Science degree as a licensed Physical
Therapist Assistant. She later went on to get her Bachelor’s degree in Healthcare
Mgmt. and Admin. In 2007. She currently resides in Louisville with her husband,
Doug, and her two sons, Chase and Logan. In her free time, Tracy enjoys watching
her boys participate in “Louisville” sports, hunting with her husband and boys and
spending time with family.
Alisa Prosser, P.T.A.
Alisa graduated from Stark State College in 2018 with an Associate of Science Degree as a licensed Physical Therapist Assistant. She currently resides in Jackson Township with her husband, Drew, and her two sons, Nico and Joey. In her free time, Alisa enjoys spending time with her family and friends, cooking, listening to live music, and traveling.
Diane Blandford, Front Office Coordinator
Diane graduated from Louisville High School in 2002. She currently lives in Louisville with her husband, Brett, and her daughter, Sadie, on their small hobby farm, which includes goats and chickens. In her free time, she enjoys helping her daughter with her 4H projects, crocheting, baking, crafting, and reading.
